San Antonio TRT providers bill in two broadly different ways, and the difference is not simply price. Membership and pay-per-visit distribute cost across the year differently and suit different patterns of use. Here is how to work out which fits your situation.
Updated October 2026.
Testosterone replacement therapy may be offered through membership, subscription or itemized billing arrangements, with different services included in each quote. Neither structure is inherently cheaper. What a year costs depend on which services a given quote covers and how much care you use, which is why comparing monthly figures between providers rarely answers the question.
Monthly TRT prices can be difficult to compare because different providers may include different services. A quote covering medication alone is not directly comparable to one that also includes consultations and laboratory monitoring.
This covers how the two models work, what each typically includes, where the break-even sits, and the questions that make a quote meaningful.
The Two Billing Models
Understanding the structure matters more than the headline number, because the structure determines what happens when your usage changes.
Feature | Pay-per-visit | Membership or subscription |
How you are charged | Separately for each consultation, the medication and each laboratory panel | A recurring monthly amount covering a defined set of services |
Cost in a quiet month | Appointment and testing charges may be lower, but medication expenses can continue | Recurring fee continues; separately billed services may add costs |
Cost during dose adjustment | Additional visits and tests may increase the total | Additional services may be covered up to the plan's limits |
Predictability | Depends on services used and recurring expenses | More predictable for included services; extra charges may apply |
What to check | Which items are billed separately and at what rate | Exactly which services are included and how many |
Commitment | Depends on the provider's billing and cancellation terms | May involve a minimum term or notice period |
Pay-per-visit costs may increase when additional appointments or tests are needed, while membership costs depend on which services the recurring fee includes. Some memberships exclude medication, laboratory testing or visits beyond a set number, and some pay-per-visit arrangements still carry a recurring medication cost in months with no appointment.
Membership may offer more predictable costs for included services, while pay-per-visit costs depend on the appointments, tests and medication billed separately. The less expensive option depends on the actual quote and expected care needs.

Where the Break-Even Sits
A useful comparison is the estimated annual cost under each billing arrangement.
Membership is worth more than pay-per-visit when the services it covers exceed what you would otherwise buy separately. That depends on three things:
The number of consultations expected during the first year and any treatment adjustments
How often laboratory monitoring is repeated, which varies with the treatment stage
Whether the medication is included in the membership or billed separately on top
The first year and later years usually differ. Starting treatment involves more consultations and more testing than maintaining it, so a membership that makes sense in year one may not in year three, and a pay-per-visit arrangement that looks cheap at the outset may not be during the adjustment period.
Which means the useful question is not which is cheaper, but which is cheaper for the year you are about to have.
A Worked Example
The figures below are invented round numbers used to demonstrate how the arithmetic works. They are not quotes, not THRYVE prices, and not representative of any San Antonio provider. Substitute the actual figures from quotes you receive.
Take two hypothetical arrangements. Provider A bill per visit. Provider B offers membership that includes a set number of services and bills beyond that at a reduced rate. Assume the medication costs the same in both cases and is billed separately by each.
Item | Provider A, pay-per-visit | Provider B, membership |
Recurring fee | None | $150 per month, $1,800 per year |
Initial evaluation | $250 one time | Included |
Baseline laboratory panel | $200 one time | Included |
Follow-up consultation | $150 each | Four included per year, then $100 each |
Monitoring laboratory panel | $150 each | Three included per year, then $100 each |
Scenario one: a settled year
Four follow-up consultations and three monitoring panels.
Provider A: $250 plus $200 plus four consultations at $150 plus three panels at $150, totaling $1,500.
Provider B: $1,800, with everything above included and no overage.
Pay-per-visit is $300 cheaper.
Scenario two: a year with adjustment
Eight follow-up consultations and five monitoring panels.
Provider A: $250 plus $200 plus eight consultations at $150 plus five panels at $150, totaling $2,400.
Provider B: $1,800, plus four additional consultations at $100 and two additional panels at $100, totaling $2,400.
At this level of use the two arrangements cost the same in this example. This is the crossover point, and it exists because Provider B charges less for additional services than Provider A charges for all of them.
Scenario three: a heavier year
Ten follow-up consultations and six monitoring panels.
Provider A: $250 plus $200 plus tens at $150 plus six at $150, totaling $2,850.
Provider B: $1,800, plus six additional consultations at $100 and three additional panels at $100, totaling $2,700.
Now the membership is $150 cheaper, and the gap widens with further use.
What the example shows
The monthly fee alone does not determine which is cheaper
The crossover depends on two things: how many services the membership includes, and what it charges beyond them
Below the crossover, pay-per-visit costs less; above it, the membership does
A membership without a stated overage rate cannot be compared at all, because the figure beyond its limits is unknown
The fourth point is the practical one. Ask what membership charges for services beyond those included, because without that figure the arithmetic above cannot be completed for any real quote.
Run the same calculation with the actual figures from quotes you receive, using a realistic estimate of consultations and panels for your stage of treatment.
What Published Sources Report on Medication Costs
Separately from the billing model, the medication itself has a cost. Published sources report generic injectable testosterone in a range of roughly twenty to one hundred dollars per month for the medication alone. Other preparations may have different medication costs depending on the product, quantity and pharmacy.
Those figures describe medication only. They exclude consultations, laboratory work and monitoring, which is precisely what the billing model determines, and the exact product, quantity and pharmacy affect what any individual pays.
Our guide to testosterone delivery methods covers how preparation affects more than cost.
Does Insurance Change the Picture?
Sometimes substantially, it interacts with the billing model rather than sitting alongside it.
Where testosterone deficiency has been diagnosed to the clinical standard, commercial insurance may cover generic testosterone. A diagnosis does not by itself establish coverage: that depends on the specific plan, the medication and the documentation the plan requires. Many specialized men's health clinics operate outside insurance entirely and do not bill it for consultations or medication, though laboratory work may still be separately billable.
A membership figure and an insured copay are not comparable numbers. One is the total cost of a bundle of services; the other is your share of one component. Comparing them directly produces a misleading answer in either direction.
Our guide to TRT benefits, risks and candidacy covers what the diagnostic standard involves, since coverage generally depends on it.

What Should Be Included Either Way
Whichever model a provider uses, certain components are part of responsible treatment rather than optional extras.
An initial evaluation establishing whether treatment is appropriate, before anything is prescribed
Diagnostic confirmation, including repeat early-morning testosterone measurements when indicated by clinical guidelines, followed by appropriate pretreatment assessment and laboratory testing
Ongoing monitoring, including red blood cell concentration, which is standard during testosterone therapy
Follow-up consultations, particularly during the first year and after any change
A defined plan for what happens if the dose or preparation needs to change
The Endocrine Society's clinical practice guideline outlines the importance of confirming testosterone deficiency before treatment and monitoring patients during testosterone therapy.
A quote that omits monitoring is not a cheaper version of the same service. It is a different service, and the difference matters clinically rather than only financially.

TRT in San Antonio: What the Local Market Looks Like
San Antonio residents can compare several types of providers offering testosterone evaluation and treatment services. Providers operate across the metro, including in the Stone Oak and Alamo Ranch areas, along the Loop 1604 and I-10 corridors, and in the South Texas Medical Center area where several larger practices and urology groups are based.
Different provider categories tend toward different billing structures, which is worth knowing when comparing quotes. Confirm which structure a specific provider uses rather than assuming it from the category.
Categories of provider you will encounter
Dedicated men's health and low-T clinics, where membership, package and itemized arrangements may be available
Urology practices, where billing and insurance participation depend on the individual practice
Primary care and internal medicine, where TRT sits alongside other care
Med spas and wellness clinics, where pricing models vary considerably
Telehealth providers, including those offering at-home laboratory collection
THRYVE Wellness Clinic is a provider-led telehealth service rather than a walk-in clinic. Evaluation, consultations and results review happen remotely, and at-home blood collection may be available in supported San Antonio service areas when appropriate for the tests ordered.
What Changes the Total Later
Several things shift the cost after the first quote, and they affect the two models differently.
Changing preparation
Switching between injectable, topical and other preparations may change medication expenses. The effect on total cost depends on the new product and whether medication is included in the billing arrangement. Under pay-per-visit that change is immediate and visible. Under membership it depends on whether medication is included or billed separately, which is worth establishing before a switch rather than after.
A period of dose adjustment
Adjustment means more consultations and more laboratory work over a few months. This is the clearest case where membership and pay-per-visit diverge, and it is also the period when someone is least able to predict how long it will last.
Monitoring settling into a routine
Once a regimen is established, testing frequency typically reduces. A membership prized around the intensity of early treatment may be covering less than it did, which is a reasonable moment to review the arrangement rather than let it continue unexamined.
Introductory pricing ending
Where a membership starts at a promotional rate, the ongoing rate is the one that matters for an annual figure. Establish both at the outset, since the promotional figure is the one usually advertised.
Questions That Make Two Quotes Comparable
Six questions. The first three apply to any model and the last three to memberships specifically.
Does this figure include the medication, or is medication billed separately
How many consultations and how much laboratory work does it cover in a year
What is the expected first-year total, including the initial evaluation
What happens if I need more visits than the membership includes
Is there a minimum term, and what notice applies if I stop
What happens to the price after any introductory period
The third question does most of the work, because the first year is the one with the evaluation and the adjustment period in it. A monthly figure without a first-year total omits the part that is hardest to estimate.

Frequently Asked Questions
How expensive is TRT per month?
It depends on the preparation and the billing model. Published sources report generic injectable testosterone in the range of roughly $20 to $100 per month for the medication alone, with other preparations varying by product, quantity and pharmacy. Those figures exclude consultations and laboratory monitoring, which a membership may include, and pay-per-visit arrangement bills separately. Ask for a first-year total rather than a monthly figure.
How much is three months of testosterone?
Three months of testosterone medication does not have one fixed price. The total depends on the prescribed preparation, quantity, pharmacy and whether the medication is billed separately or included in a care arrangement.
Is TRT membership cheaper than paying per visit?
Not inherently. Membership costs less when you use more of what it includes, and more when you use less. Treatment tends to involve more consultations and testing in the first year and during any adjustment, and fewer once a regimen is established. Work out which year you are about to have rather than comparing monthly figures.
Can I get testosterone without seeing a doctor?
No. Testosterone is a Schedule III controlled substance in the United States and requires a prescription from a licensed provider, which requires a clinical evaluation. Providers offering it without an evaluation are not following the expected standard. Telehealth consultations are a legitimate route to that evaluation, but an evaluation still must happen.
Does insurance cover TRT in Texas?
It may, where testosterone deficiency has been diagnosed to the clinical standard. Coverage varies by plan and by what is being billed, and many specialized clinics operate outside insurance entirely. Consultations, laboratory testing and medication may each be treated differently by the same plan. Verify with your own insurer directly.
Final Thoughts
Membership and pay-per-visit are not two prices for one thing. They are two ways of distributing the same costs across a year, and they suit different patterns of use.
Pay-per-visit and membership arrangements distribute expenses differently, but neither guarantees a lower total cost. Which suits a situation depends on what a specific quote includes and how much care is expected, rather than on which monthly figure looks lower.
Cost comparisons can help with planning, but treatment decisions should begin with an appropriate clinical evaluation.
